Booking an eye examination or contact lens check online

For new patients…

• Click the ‘New’ button.
• Enter your details into the spaces provided. All fields must be filled unless stated otherwise.
• Click the ‘Next’ button once you have completed the form.
• You may now select the appointment type and date/time from the available appointments.

For existing patients…

• You will need your Patient Number, this will be on your reminder correspondence.
  • Please contact us if you are a patient and would like to know your Patient Number.
• Click the ‘Existing’ button.
• Enter your Patient Number, date of birth and the security code.
• You may now select the appointment type and date/time from the available appointments.

Booking the appointment…

• Select the available appointment type from the drop-down list
• Select the optometrist you would like to see, if you do not have preference select ‘[ANY]’.
• Select the date and then convenient time.
• Check the details of the appointment chosen and press ‘Confirm’ to submit the appointment.
• Once confirmed you will received online you will receive an email confirmation or an Eyediology team member will contact you to confirm the appointment.

If the appointment you require is not listed here, call Eyediology on 020 7377 2020, during working hours, as there may be other appointments and dates that are not available online, or for any other assistance.
